How do I get a certified copy of the Notice to Retake Surname?

• Montgomery County - You must include a money order in the amount of $5.25 for each certified document you are requesting. Include a self-addressed, stamped envelope and cover letters stating you are requesting a certified copy.

• Chester County - You must include a money order in the amount of $6.75 for each certified document you are requesting. Include a self-addressed, stamped envelope and cover letters stating you are requesting a certified copy.

• If you only need a copy (NOT CERTIFIED) of the original Notice to Retake Surname, include a self-addressed, stamped envelope and a cover letter stating you are requesting a copy signed by the Judge to be returned to you.
