The Praecipe to Transmit does ask for the "date filed." However, we have been instructing our clients to file the Affidavit and Waiver documents at the same time they file the Praecipe to Transmit, rather than as an additional filing step before filing the Praecipe. The date that you sign the Affidavit and Waiver is the date you may place in the Praecipe to Transmit. We have never been challenged by the court.
